Wintringham is a specialist not-for-profit company, which provides aged care services to those who are financially disadvantaged and have previously been homeless or are at risk of becoming homeless. Wintringham is seeking a motivated and enthusiastic person with a commitment to social justice to join our multi-disciplinary Specialist Aged Care Consultancy Service, for prisons. This lead role will assist clients living in prison to identify their transitional plan into the community. You will be helping address issues of social isolation for program clients and assist clients to maintain or improve their accommodation.
Working with the Wintringham Clinical team, this hands on role will see you helping address issues of social isolation for prison exiting clients and assist clients to maintain or improve their accommodation. As the key ‘go to’ person for prison and site based nursing staff, you will have an excellent understanding of current best practice guidelines in aged care and disability and be confident in providing advice and guidance. The successful applicant must have a minimum tertiary qualification in Social Work with previous experience in a similar role (preferably within Aged Care). Candidates with a detailed knowledge of alcohol dependence and associated behaviour will be looked on favorably.
The overall responsibilities include undertaking initial assessments for clients, with aged care needs, who will be transitioned into the community, assisting those clients access the support they need and ensuring accurate recording and documenting of data and issues faced in supporting “to be released:” prisoners - for future evaluation by Corrections Victoria. You will have excellent working knowledge of the My Aged Care and National Disability Frameworks, exceptional communication skills, a non-judgemental attitude and the ability to liaise with people from varying socio-economic backgrounds.
