Social Worker

Part-Time Fixed Term (0.4 EFT to 30 June 2018)

Darebin Community Health (DCH) is an accredited community health service with an independent Board of Directors. It is located in the northern suburbs of Melbourne and works with a diverse socioeconomic and multicultural community. DCH provides a wide range of allied health, dental, medical, social support and health promotion services to the Darebin Community.

Darebin Community Health is currently looking for a Social Worker to join the Older Adult Program. The Older Adult Program provides Allied Health services within Darebin and the north metro region with a focus on people over 65 years.

The successful candidate will provide high quality social work services to clients who are eligible for the Commonwealth Home Support Program (CHSP). The role involves assisting clients to access appropriate services including My Aged Care and providing social work support to a multidisciplinary allied health team in the form of episodic case management, care coordination, counselling and advocacy.

Your Key Responsibilities for the role will include:

  • Support the provision of social work services that are safe and high quality
  • Provide such services within a multidisciplinary team framework
  • Develop social work service within the Older Adult Program

To succeed in this role you will need the following skills:

  • Ability to provide social work services to older people from diverse backgrounds with complex needs
  • Commitment to a social model of health consistent with community health philosophy
  • Ability to organise work both cooperatively and independently within a multidisciplinary team
